| Description | The first considered diagnosis of a condition or illness, which may be changed as further information becomes available and is analysed. |
| Use | To record an initial working diagnosis of a condition or illness, which may be changed as further information becomes available and is analysed. A provisional diagnosis is used to set in motion the first stage of treatment and management and may be revised as needed, until a formal diagnosis is confirmed. This archetype is purposefully designed as a separate concept to the Problem/Diagnosis archetype to ensure that initial, working, or unconfirmed, diagnoses are not mixed with confirmed diagnoses and perpetuated as part of ongoing shared problem lists. |
| Misuse | Not to be used to record a confirmed diagnosis. Use EVALUATION.problem_diagnosis instead. Not to be used within persisting and long-term Problem lists. |
| Purpose | To record the first considered diagnosis of a condition or illness, which may be changed as further information becomes available and analysed. It is used as a working diagnosis until a formal confirmation can be made. |
